New patient portal

Dear Sir, Madam,

Soon you will be able to access your medical file online using your home computer, smartphone or tablet.

Your medical file contains:

* Advice from your family doctor

* Test results

* Overview of your prescriptions

To make it simple for you to access this information, our medical practice will begin using a new Patient Portal. In addition to viewing your medical file, you can use this new portal to:

* Ask a question to your family doctor (e-consult)

* Request a prescription refill

* Make an appointment

* Receive a message from your healthcare provider

On 12 November 2020 our new Patient Portal will be available and the current portal no longer accessible. We are migrating information and expect your medical file to be visible online during December.

Using the new Patient Portal.

To use this new Patient Portal, unfortunately you do need to make a new account. This is necessary to meet heightened government security requirements. You can create your new account using the new Portal. Existing accounts from our current system will be closed and deleted by us.

You can find the new Portal on/after 12 November 2020 on our website: https://www.hapmb.nl/

directly via: https://hapmb.uwzorgonline.nl/l

or using the app from this website: https://uwzorgonline.nl/

Is your personal information safe in this portal?
Only you, your family doctor and employees of the doctor’s practice have access to your data. The doctor and employees of the doctor’s practice have a professional responsibility to keep your information confidential. You can see your own medical records within your personal portal. Do not give your access codes to other people.

Be careful sharing your medical information with others.
Never give your medical records to anyone without good reason. Before sharing information with a family member or close friend, think carefully whether you are comfortable with someone else having access to your medical file.
Sometimes insurance companies, municipalities, or other companies ask for information about your health. Often you have no obligation to share this information. In case you decide to share information, do not give your complete medical file. Share only the necessary information.

Do you have questions or doubts about giving medical information to a third party? Consult with your doctor or ask the assistant. They can help you.

Kind regards.
